Forms for Foster Care Services


The following forms are to be used by the foster care family or foster-to-adopt family while any child is in a foster care placement with the family. If you have any questions about the forms, please contact your API Case Manager. All forms, unless otherwise specified, must be turned in by the 5th of the month following the service month (For example, September forms must be turned in by October 5).


Accounting of Child's Money  ~ This form is to be used when the child earns or receives any money from the foster family (including allowances and gifts), jobs, caseworkers, and other individuals. Gift cards are treated as money. When the child enters your home, if they have any money or gift cards, this amount will be entered as the beginning balance. If the child does not have any money or gift cards, then once the child does, the beginning balance will start. The foster parent is required to document all incoming and outgoing amounts including the receipt and use of gift cards on this form. A copy of this form is to be turned in every month to the API Office Manager by the 5th of the month following the service month. If the child leaves the placement, then the form must be up to date and any money or gift cards must accompany the form and be given to the API Case Manager at the discharge of the child from the home.

Adverse Reactions and Side Effects of Medication ~ This form is to be used when you make an error in providing medication to the child - including forgetting a dose, giving the wrong dosage or giving the wrong medication. It is also to be used to document any adverse reactions or side effects of the medication given. If it is necessary to contact the prescribing physician or pharmacy, you must document the name, contact time and instructions given to you by the appropriate medical personnel. A copy of this form must be turned in every month to the API Office Manager by the 5th of the month following the service month.

Children's Rights and Responsibilities ~ This document is to be signed by the child and the foster parents on the day of placement and given to the API Case Manager. This is a one-time form.

Clothing and Personal Items Inventory ~ This form is to be used at the beginning of the placement to document all of the items the child brings into your home. It must be turned in to the API Office Manager within 5 days of placement. Then, it is to be submitted quarterly to the API Office Manager to document the clothing and personal items the child maintains. The foster family is responsible for purchasing clothing and personal items for the child, which may include but are not limited to gifts, school items, personal hygiene items, decor for the child's room.

Disaster Plan ~ This document must be filled out during the verification process and anytime you change your disaster plan, evacuation methods or emergency contacts.It must be updated every 2 years or at the above mentioned times. It is to be turned in to the API Office Manager.

Life and Social Skills Training ~ This form is to be used to document the life and social skills activities you participate in each month. The foster parent needs to provide at least one social event, one cultural event an one life skills event each month for each child. These may include but are not limited to attending church services, interacting with other children in after school care, attending parties, visiting museums or festivals, making food, helping with chores, etc. A copy of this form is to be turned in every month to the API Office Manager by the 5th of the month following the service month.

Medical and Dental Treatment Forms ~ This form is to be used at all doctor's appointments, including medical check-ups, acute care visits, follow-ups. This includes medical, vision, hearing and dental appointments.  Take this form with you to the appointment and have the doctor fill it out while you are there. A copy of this form is to be turned in every month to the API Office Manager by the 5th of the month following the service month.

Medication Log for Prescribed Medications ~ This form is to be used to document all administering of prescribed medications. A copy of this form is to be turned in every month to the API Office Manager by the 5th of the month following the service month.

Medication Log for Non-Prescribed/OTC Medications ~ This form is to be used to document all administering of non-prescribed or over the counter medications. This does not include vitamins.  A copy of this form is to be turned in every month to the API Office Manager by the 5th of the month following the service month.

Monthly Caregiver Log ~ This form is to be used to record the foster parents observations of the child's overall behavior for the month. It needs to cover each area described so that the API Case Manager can ascertain how the child is adjusting to the placement. A copy of this form is to be turned in every month to the API Office Manager by the 5th of the month following the service month.

Recreation Schedule ~ This is a monthly calendar that documents the foster family's recreation schedule. The foster family needs to provide recreational activities for the child on a daily basis that are appropriate to the age and development of the child taking into consideration their abilities and talents and desires as well as the weather. This may include such activities as reading, playing outside, watching a movie, etc. A copy of this form is to be turned in every month to the API Office Manager by the 5th of the month following the service month.

Request for Approval to Travel ~ This form is to be used when the family plans to travel for more than 48 hours outside of the county of residence. This form must be turned in as soon as you know about the travel, prior to its occurrence. This form must also be used if the child is traveling without the family or staying anywhere else for more than 48 hours.  In some cases, travel must be approved by CPS and the courts and requires at least a 2 week notice (for instance: out of state travel and out of country travel).

Search Form ~ This form is to be used to document any necessary searches of the child or their belongings. Searches may only be conducted according to the instructions on the form and if there is a suspicion that the child may harm themselves or others. While you must notify the API Case Manager immediately after the search is conducted, a copy of this form is to be turned in every month to the API Office Manager by the 5th of the month following the service month.

Serious Incident Report ~ This form is to be used to document the listed serious incidents that might occur to a child while in the foster home. When it doubt as to whether it is serious, document anyway. A copy of this document must be provided to the API Office Manager within 24 hours of the incident or sooner.

Short Personal Restraint Form ~ This form is to document any use of a short personal restraint and the discussion with the child about the short personal restraint. If an injury occurs during a short personal restraint, then a Serious Incident Report must also be completed. A copy of this document must be provided to the API Office Manager by the 5th of the month following the service month, unless there is an injury, in which case the form must be provided within 24 hours.

Sibling Visit Report ~ This form is to be used if the child has sibling visits with sibling during any month. A copy of this form is to be turned in every month to the API Office Manager by the 5th of the month following the service month.
